Welcome to the NAWS Campus Club Services!

To streamline the check-in process for Club Services we have posted some of the forms you will need to fill out below.

The process is to call or e-mail us to make a reservation for Overnight Stays, Day Camp, Day Spa, or Training Classes and we will make the reservation and direct you to the forms listed below so you can complete them and either scan/fax them to us in advance at 708-478-5803, or bring them with you at check in.

In addition to the forms below, all pets that utilize Club Services will need to be up to date on vaccinations, flea prevention, and in the case of Day Camp a negative fecal test.  If you do not have your pet's current veterinary history from your veterinarian, please request a copy as soon as possible and you may fax it to us in advance, but please bring the copy with you even if it is faxed to us.
